Confirmation Forms and Floor Plans
To confirm a reservation “HOLD”, please select and complete the applicable Confirmation Form below.
When returning the completed Confirmation Form, please save the form using the event name, and the event date. For example save as: StudentOpenHouse_6-7-17.

All Confirmation Forms are fillable PDF's (allowing you to type directly into the form input fields). Fillable forms work best using the Chrome browser, and may need to be downloaded when using Firefox.

Fixed Floor Plans
This section has been updated to show the new "fixed" layout options, and the maximum room capacity for the East Dining Room, the Faculty Dining Room, and the Rehearsal Hall (MU-330) located in the Music Building. Note: The furniture layouts for each venue cannot be altered; all campus reservation requests must select one of the approved "fixed" layout options.
*Once you have chosen the layout & entered the information required please attach the floor plan with the confirmation form to the "HOLD" email trail to finalize the reservation.
